You must have Adobe Reader (or MS Word if so indicated) installed. Click on the link to download and install Adobe 2. Select a file below  3. The Adobe files (PDF) can only be filled out and printed. You cannot save or email them with your data filled out 4. MS Word files (DOC) can be printed, emailed, and save with your data filled out. We are currently converting the remainder to DOC format as well. If you don't have Word, download and install the free viewer. a. When you click on the DOC link below, you will immediately get an "Open or Save" dialog box. b. We recommend you Save It to your computer and open it with your own version of Word. After filling out the data fields, you can email it, print it, and/or save it filled out. If you save it, you may have to save it under a different file name.
